A 61-year-old man has died in a tragic incident at a medical facility in Westbury, Long Island, after being violently pulled into an active MRI machine by a metal chain he was wearing, according to New York police.

The fatal accident occurred on July 16 at Nassau Open MRI, a diagnostic imaging center. Authorities say the man entered the MRI room without authorization while the machine was still operational a critical safety breach in a highly magnetized environment.

The chain around his neck was reportedly made of metal and became magnetically attracted to the machine with extreme force, pulling the man into the equipment and causing fatal injuries.

Emergency responders arrived at the scene, but the man was later pronounced dead.

Police are investigating how the man gained access to the MRI room and whether staff or safety protocols failed to prevent the incident.

The name of the victim has not yet been released.
